Adding a List of Entries to a Form

Here is an example of a form for a company that allows you to enter multiple employee records.

Use FORMgen to create a form with the fields:
- PersonNameField called "company"
- StateField called "state"

Then add the following fields for the employee records:
- PersonNameField called "name1"
- EmailAddrField called "email1"
- PhoneField called "phone1"
- PersonNameField called "name2"
- EmailAddrField called "email2"
- PhoneField called "phone2"
etc...
